Ok, I almost started to believe in magic.

I kid you not, here is a screenshot.

The file is just two copies of the same file from the distribution.

Answer:

strace (and some RTFM) fixes everything, fail2ban-regex pulls files from /etc/fail2ban/filter.d/ if not preceded by "./"

Took me 15 mins of WTF'ing to find out.
