Okay, if someone I really don't know asks I always lie.
They don't really want to know, they're just going through the pleasantries.
I usually just share with people who I assume can at least relate and understand to a degree and actually want to know

Depends on who is asking, but if I'm feeling miserable and don't want to talk about, but want to hint at it, I'll answer with: "alive"
If I don't know the person well enough and don't want to make myself vulnerable I'll either dodge the question, lie ("I'm ok") or pull out the classic "muss ja, ne?".
Seldomly, when I have enough energy in such a situation and know the person good enough, I'll answer honestly with what bothers me. And that usually helps a bit, even by just getting it out.
Although I'm currently in the privileged position of not needing that too often lately, as my situation has stabilized to a point. I've mostly come to terms with my difficulties, it's a heck of a journey, but definitely in a positive direction.
