I wasn't even going to write public posts anymore but one day of executive orders & suddenly doctors are refusing to prescribe contraceptives? Or think they are enjoined from doing so?

I get not taking new clients if I was a reproductive healthcare provider right now, that's a different matter entirely.

But you've a responsibility ethically to those you've taken on.

Yes there is always the right not to prescribe, or therapeutic privilege. True.

But absent any bigoted law intended to strip someone of their autonomy, to harm them, to unjustly attack them, would the oral contraceptive be the treatment of choice?

This goes for doctors providing care too.

You must continue to practice EVIDENCE based medicine. Your ethical duties are to your patient, their autonomy, doing right by them, and doing justly by them while not harming (or mitigating harm to) them. That's the four ethical pillars in a nutshell.

If you don't advocate for such, your patients will lose their rights, their medications, their dignity, their respect.

So keep writing for oral contraceptives, mentioning the treatment for an ectopic being an abortion, and continue to force hospital (facility) attorneys to enjoin you from doing so.

For example, if an oral contraceptive won't cause significant harm TO THE PATIENT (not to some mysterious magical theoretical cyst), and its indicated for PCOS, severe menorrhagia, etc, you are obligated by your ethical duties to treat them.

Let the hospital attorneys enjoin you from doing so or the pharmacist refuse to fill it or the insurance refuse to cover it.

Do not comply in advance if you give a single square shit about a single patient.

The absolute worst thing that can be done right now is to cede ground in advance. Whether this is tug'o'war or Capitol Hill negotiations or anything for that matter, complying in advance sets up the zero sum fash type to take that ground and divert focus to more ground, keeping you on your back foot the entire struggle.

Call them out if they comply in advance.

Dont let them off the hook.

Dont care the state.

A medical indication for a contraceptive -- PCOS, whatever -- doesnt change because a bigot litigates it so.
